Guest Liability Release

Upon registering and initialing below, I hereby relieve the studio, its owners, and staff of any liability for injury or harm which may occur from using the facility and any equipment or services offered by the facility. I agree to and understand that I am using the equipment and facility at my own risk and that I assume all responsibilities and/or claims to injuries or harm. Furthermore, I understand and consent that the studio owners and/or staff are not responsible for any lost, stolen, or damaged personal properties including automobiles, electronic devices, wallets, and the like. I further acknowledge and agree to fully follow all posted notices including warning signs and rules.
